Responsibilities
- Provide leadership in developing, implementing, revising, and reporting the case management program
- Serve as a liaison among the patient, family, nurse, physicians, multidisciplinary team, and healthcare benefactor to optimize outcomes
- Act as a consultant to the care team on specific patient items requiring coordination
- Coordinate a continuum of care for a defined patient population from pre-admission through post-discharge
- Ensure patients and families move efficiently through the hospitalization process and discharge planning
- Document case management activities, track outcomes, and communicate the care plan to the team
Requirements
- Active RN license to practice in California
- Experience or demonstrated ability in case management or care coordination
- Strong communication, organizational, and documentation skills
- Ability to work effectively in an inpatient/acute care environment
- Flexibility to participate in a 13-week, travel assignment
