Responsibilities
- Administer prescribed medications and treatments under supervision of an RN or physician
- Monitor patient condition, document vital signs, and report changes promptly
- Provide ADL assistance and comfort measures to residents as indicated by care plans
- Assist with wound care, injections, and routine nursing procedures per protocol
- Collaborate with interdisciplinary team to update and implement individualized care plans
- Maintain accurate documentation in the electronic health record and adhere to safety and infection control policies
Requirements
- Active Licensed Practical Nurse license in New Mexico, in good standing
- Graduate of an approved practical nursing program (LPN degree)
- Long-term care or skilled nursing experience preferred
- Ability to work the 06:00–18:30 local shift and meet a guaranteed 36 hours per week
- Strong communication and teamwork skills; proficient in basic electronic health record documentation
- Ability to pass standard credentialing and background checks required for travel assignments
