Responsibilities
- Deliver physical therapy services within the legal scope of practice for the state of practice, maintaining required licensure and certifications.
- Conduct initial evaluations, write progress notes, perform 30-day reassessments, and 60-day recertifications in alignment with Medicare, state, and agency standards.
- Develop, implement, and update individualized plans of care aligned with physician orders and patient needs; deliver skilled interventions and maintenance therapy.
- Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides while coordinating with physicians and the interdisciplinary team on treatment adjustments.
- Educate patients and caregivers on home exercise programs, mobility strategies, and safety to promote independence.
- Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency timeframes; apply ICD-10 coding for accurate documentation.
- Participate in in-service programs, Quality Assessment and Performance Improvement (QAPI) activities, and interdisciplinary meetings; adhere to HIPAA and privacy standards.
- Maintain infection control, safety, and risk prevention protocols; document interventions and care coordination promptly; use organizational technology systems effectively.
Requirements
- Current license as a Physical Therapist in the state of practice (New Mexico).
- Graduate of an accredited physical therapist curriculum approved by CAPTE or equivalent.
- Two years of clinical experience preferred; home health experience a plus but not required.
- Current CPR certification; ability to meet Medicare/Medicaid Conditions of Participation and agency policies.
- Basic computer proficiency; willingness to learn EMR systems and OASIS documentation; reliable transportation with valid driver’s license and auto insurance for home visits.
- Ability to work independently in varied home health settings and communicate effectively with diverse patient populations.
