Responsibilities
- Evaluate residents to determine physical therapy needs and prognosis
- Develop, implement, and monitor individualized treatment plans aligned with goals
- Administer evidence-based therapies including therapeutic exercises, gait training, and modalities as appropriate
- Document progress and outcomes; communicate findings to the care team and family as needed
- Collaborate with the interdisciplinary team to support discharge planning, equipment needs, and safety
- Ensure patient safety, adherence to standards, and quality of care during therapy sessions
Requirements
- Active Physical Therapy license in Pennsylvania
- Graduation from an accredited physical therapy program (DPT or equivalent)
- Ability to work in a long-term care setting and meet the specified shift hours
- Strong communication, documentation, and collaboration skills
- Preferred: experience or exposure to skilled nursing facility or inpatient rehab settings
