Responsibilities
- Provide clerical support for Case Management, including processing authorizations and pre-cert reviews
- Coordinate post-acute referrals and discharge planning with interdisciplinary teams
- Deliver Medicaid notices and maintain knowledge of Medicaid/Medicare basics
- Support care transitions through referrals to home care, DME, rehab, and long-term care
- Maintain accurate documentation in EPIC to support patient care and transitions
- Communicate status and needs with PT, Interpretation Services, Social Services, Pharmacy, and Transportation as part of the care continuum
Requirements
- EPIC charting experience required
- Minimum 1 year of experience; first-timers accepted
- WA clinical license not required
- Ability to perform in a hospital-based case management clerical role
- Travel requirement: outside 50 miles; locals not accepted
