Role Specifics and Benefits:
Key responsibilities
- Conduct comprehensive PT evaluations and develop individualized, goals-based treatment plans that address musculoskeletal, neuromuscular, and functional goals.
- Implement evidence-based interventions, monitor progress, and adjust plans to maximize recovery and independence.
- Document outcomes clearly in the electronic health record and communicate findings to the care team for coordinated discharge planning.
- Educate patients and families on home exercise programs, safety, fall prevention, and strategies to maintain gains after discharge.
- Collaborate with physicians, therapists, and ancillary staff to ensure seamless, high-quality care and to support quality improvement efforts.
- Adapt to a variety of clinical settings, bringing consistency, professionalism, and a patient-centered approach to every assignment.
