Position Overview

As a Long Term Care CNA, you will focus on continuous, resident-centered care in skilled nursing facilities or extended care environments. You will provide daily living support, monitoring, and companionship to residents with chronic conditions while working closely with nursing staff to maintain comfort, safety, and adherence to individualized care plans.

Key Responsibilities

• Daily Care & Support: Assist residents with activities of daily living (ADLs), mobility, and hygiene while maintaining a clean, safe, and respectful environment.

• Monitoring & Documentation: Monitor and document vital signs, intake/output, and changes in condition accurately in the facility’s EMR.

• Care Plan Execution: Support routine procedures, therapy setups, and medication administration under direct supervision per facility policies.

• Team Collaboration: Work seamlessly alongside RNs, LPNs, and therapists to implement personalized care plans and report changes promptly.

• Resident Advocacy: Support residents' emotional well-being and offer basic guidance to residents and their families.

Qualifications & Requirements

Education:

• High school diploma or GED equivalent.

• Completion of a state-approved CNA program with competency evaluation.

Licensure & Certifications:

• Current State CNA Certification in good standing (Vermont or compact equivalent).

• Active CPR / BLS Certification.

Experience & Skills:

• Demonstrated experience providing resident-focused care in SNF or long-term care settings.

• Experience in EMR charting and vital sign recording.

• Strong observational, interpersonal, and communication skills.

• Flexibility to handle any shifts on call with a 24-hour notice.

Preferred Experience:

• Background in wound care assistance or fall prevention protocols.

• Strong knowledge of infection control practices.

• Bilingual capabilities (Spanish or other languages a plus).
