Responsibilities
- Provide direct patient nursing care to residents under supervision of a registered nurse
- Administer prescribed medications and document administration per policy and procedure
- Monitor vital signs, assess patient condition changes, and escalate as needed
- Perform routine nursing procedures, wound care, ADLs assistance, and infection control
- Maintain accurate health records and contribute to care planning and interdisciplinary communication
- Promote patient safety, assist with transfers and mobility, and uphold a clean, organized care environment
Requirements
- Active LPN license (state-specific licensure as required)
- Current CPR/BLS certification for healthcare providers
- Ability to work the stated evenings shift (15:00–23:00)
- Experience in a skilled nursing or long-term care setting preferred
- Strong communication and teamwork skills
- Willingness to travel for a travel assignment
