Responsibilities
- Provide direct nursing care to residents per individualized care plans, including routine assessments and care tasks.
- Administer medications and treatments per physician orders, within scope and facility policy, using correct protocols.
- Monitor vital signs, assess changes in resident condition, and document findings accurately; escalate issues promptly.
- Collaborate with an interdisciplinary team (including CNAs, therapists, and physicians) to coordinate care and contribute to discharge planning.
- Maintain a safe, clean environment; follow infection control, fall prevention, and safety protocols to protect residents.
- Complete accurate charting and thorough shift handoffs to ensure continuity of care and proper documentation.

Requirements
- Active Licensed Practical Nurse license in New Mexico
- Ability to practice in a long-term care setting
