Responsibilities
- Conduct comprehensive OT evaluations and develop patient-centered treatment plans for home health clients, focusing on activities of daily living, mobility, cognition, and safety.
- Implement and adjust therapy plans, document progress, and communicate changes to the care team.
- Educate patients and families on home safety, ADL strategies, energy conservation, and use of assistive devices.
- Collaborate with an interdisciplinary team (nurses, physicians, and other therapists) to coordinate care and support discharge planning.
- Assess home environments for accessibility needs and recommend equipment or environmental modifications as appropriate.
- Deliver OT services in patients’ homes and ensure timely, accurate documentation to support continuity of care.
Requirements
- Active OT license in South Dakota or eligibility for SD licensure
- Degree from an accredited occupational therapy program (Master’s or Doctoral level)
- Experience in acute care or home health settings preferred; not required, but considered
- Ability to work in a home-based setting and to travel to patient homes within the service area
- Reliable transportation and a valid driver’s license
