Responsibilities
- Implement prescribed PT treatments under the supervision of a licensed PT, including therapeutic exercises, gait and transfer training, balance activities, and neuromuscular re-education to improve function and mobility.
- Assist with patient handling, transfers, bed mobility, activities of daily living (ADLs) training, and safety education; support patient participation and family engagement as appropriate.
- Monitor patient responses during therapy sessions, observe signs of fatigue, pain, or adverse events, and communicate findings promptly to the supervising PT; participate in plan-of-care adjustments as directed.
- Document therapy sessions, progress toward goals, assistive device use, and equipment cleaning in the chart per facility policy; ensure accuracy and timeliness of documentation.
- Prepare and organize treatment areas, ensure proper sanitation, and maintain a safe environment; manage treatment supplies and equipment.
- Collaborate with an interdisciplinary team (nursing, OT, speech therapy, and physicians) to coordinate patient care plans and discharge planning; participate in daily huddles or care coordination meetings as needed.

- Strong communication and teamwork skills to function in a multidisciplinary care environment
- Demonstrated ability to comply with facility policies and documentation standards
