Responsibilities
- Administer prescribed medications and treatments per the plan of care, adhering to facility policies and applicable regulations.
- Monitor patient conditions, assess vital signs, identify changes, and report findings promptly to the supervising RN.
- Provide direct nursing care and support activities of daily living as assigned, while maintaining patient comfort and safety.
- Document care accurately in the patient chart and ensure timely communication with the health care team to support continuity of care.
- Collaborate with RNs, physicians, and other members of the interdisciplinary team to implement and adjust care plans as needed.
- Maintain infection control practices, safety protocols, and a clean, safe environment for residents and staff.
Requirements
- Active Licensed Practical Nurse (LPN) license in good standing (state-specific licensure required as applicable).
- Current CPR/BLS certification.
- Graduation from an accredited practical nursing program (LPN diploma/degree).
- Prior LPN experience in long-term care or skilled nursing settings preferred.
- Ability to work a 40-hour week, including Day and Evening shifts, and participate in a travel assignment as needed.
- Strong communication, documentation, and teamwork skills to support resident care and regulatory compliance.
