- Travel policy: Local candidates within 75 miles may qualify for the local rate
Responsibilities
- Deliver direct nursing care to adult residents on a skilled nursing unit under supervision
- Administer medications and treatments per physician orders and facility policy
- Monitor and document patient status, vitals, pain levels, and responses to interventions
- Write and maintain accurate records in PointClick Care EMR; coordinate with the charge RN and interdisciplinary team
- Float to all units within the same building as needed; assist with activities of daily living and mobility
- Respond promptly to call bells and changes in patient condition; adhere to safety and infection control protocols
- Support residents with memory care, dementia, orthopedic and MS-related needs consistent with care plans
- Maintain a respectful, patient-centered approach and uphold regulatory standards
Requirements
- Active New York Licensed Practical Nurse (LPN) license
- BLS (basic life support) certification
- 1+ year of LPN experience, preferably in long-term care or skilled nursing settings
- Education: LPN diploma or certificate from an approved program
- Ability to work a 6:30 am – 2:30 pm shift and to float to all units within the building
- Availability to work every other weekend as required
- Proficiency with PointClick Care or willingness to learn the EMR system
