Responsibilities
- Perform comprehensive PT evaluations and re-evaluations in patients’ homes and establish a Plan of Care with measurable goals
- Implement evidence-based physical therapy interventions (therex, gait and balance training, manual therapy, modalities) and monitor progress
- Document treatment sessions, outcomes, and communicate with supervising PT and the interdisciplinary care team
- Ensure treatment plans align with patient goals, safety standards, and agency policies
- Travel to patient residences within the designated service areas and participate in occasional weekend coverage as needed to maintain caseload
Requirements
- Current California Physical Therapy License
- BLS certification
- Valid driver’s license, auto insurance, and vehicle registration
- Immunizations and screenings: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
- Drug screen and physical examination as part of onboarding
- Ability to travel to patient homes and work in a home-health setting; willingness to cover weekend shifts if required
