Responsibilities
- Coordinate patient care across disciplines, manage discharge planning, and assess admission criteria to support appropriate resource use.
- Apply InterQual and Milliman guidelines to determine level of care and plan of treatment.
- Maintain knowledge of CMS, CPT, DRG, HEDIS, and NCQA standards to ensure regulatory and payer compliance.
- Ensure accurate, timely documentation through electronic charting; uphold HIPAA and privacy standards.
- Facilitate smooth transitions of care, communicate with the interdisciplinary team, and provide patient and family education as needed.
- May float to like units within scope as needed; brief orientation will be provided.

Requirements
- Current RN license valid in New Mexico or eligible for NM/Compact licensure; pending NM/Compact licenses accepted but must be obtained prior to start.
- Minimum 1 year of RN case management experience.
- BLS certification.
- Proficiency with electronic charting; charting experience required.
- Knowledge of ICD-10 coding; familiarity with CMS, CPT, DRG, HEDIS, NCQA standards.
- Ability to work in a hospital setting and float to related units within scope; RTOs must be listed at submittal; up to 4 days auto-approved.
