Responsibilities
- Provide direct patient care, including evaluations, consultations, and treatments to improve physical performance
- Conduct individualized evaluations and design goal-directed, customized plans to enhance mobility and function
- Perform ongoing assessments, monitor progress, and adjust treatment plans as needed
- Document evaluations, plans, progress notes, and discharge summaries in a timely, accurate manner
- Collaborate with the care team to coordinate services and ensure safe, effective care delivery
- Educate patients and caregivers on home exercise programs, safety, and discharge planning
Requirements
- Ability to perform direct patient care and deliver therapy services in a hospital-related setting
- Demonstrated proficiency in patient evaluations and developing individualized treatment plans
- Strong documentation, communication, and interdisciplinary collaboration skills
- Flexibility to work a 4x10 hour shift schedule
