Responsibilities
- Conduct initial PT evaluations for residents and develop individualized treatment plans within a long-term care setting.
- Implement therapeutic exercises and interventions to improve mobility, strength, balance, and gait; address activities of daily living.
- Monitor and document patient progress, adjust treatment plans as needed, and communicate findings to the care team.
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification.
- Collaborate with an interdisciplinary team to optimize functional outcomes and coordinate discharge planning.
- Maintain accurate, timely documentation in the resident’s chart and ensure compliance with privacy and regulatory requirements.
Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Current physical therapy license in Montana or eligibility to obtain MT licensure.
- Ability to work in a long-term care facility environment with residents of diverse rehabilitation needs.
- Strong communication and documentation skills; team-oriented approach.
