Licensed Practical Nurse (LPN) – Skilled Nursing (SNF) – Lake Isabella, California, United States. Travel - temporary assignment beginning 04/13/2026 for 13 weeks, nights shift, in a hospital setting. Weekly pay range: $1,145–$1,193 per week. Guaranteed hours: 36 per week. This assignment may involve cross-training in other nursing departments as needed. The role emphasizes direct patient care under supervision of an RN and supervisory support for CNAs and unit staff.

Responsibilities
- Assess, plan, and implement nursing care for patients under the supervision of an RN.
- Administer medications and treatments in accordance with physician orders and facility policy.
- Monitor patient status, document findings in the electronic medical record, and communicate changes to the care team.
- Provide direct patient care, help with activities of daily living as appropriate, and ensure patient safety.
- Supervise CNAs and support staff to maintain continuity and quality of care.
- Collaborate with RNs and other disciplines to adjust care plans and respond to patient needs; participate in unit cross-training as required.
Requirements
- Active California LVN license (required for this role)
- Long-term care experience preferred; SNF experience required for this assignment
- Current Basic Life Support (BLS) certification
- Proficiency with Electronic Medical Records; operating knowledge preferred
- Ability to pass a nursing medication test
- Excellent English communication skills (verbal and written)
- Strong interpersonal skills and professional, tactful interaction with patients, families, staff, and regulatory reps
- Basic computer literacy
- Availability to work the night shift and participate in rotation blocks as described
