As the frontline provider for residents in long-term care, you’ll perform a wide range of responsibilities: administer medications and treatments in accordance with physician orders; monitor vital signs and health changes, promptly escalating concerns to supervising RNs; perform wound care, catheter care, and routine assessments; support daily living activities, mobility, and rehabilitation goals; assist with admission, discharge, and transfer processes; document care plans, progress notes, and critical observations in the electronic medical record with accuracy and timeliness; collaborate with interdisciplinary teams to develop and revise individualized care plans; reinforce infection control practices, safety protocols, and fall prevention strategies; engage with residents’ families to explain care plans and address questions with diplomacy and empathy; and participate in continuous quality improvement initiatives to enhance care delivery and resident satisfaction.
