Responsibilities
- Administer prescribed medications and treatments per the plan of care; document patient responses and any changes observed.
- Monitor residents’ conditions through routine assessments, vital signs, and observation; escalate changes to the supervising RN as needed.
- Provide core nursing care, assist with activities of daily living, and support routine rehabilitative activities to promote comfort and safety.
- Maintain accurate charting and MAR documentation; ensure compliance with regulatory requirements and facility policies.
- Collaborate with the interdisciplinary care team to update care plans and ensure coordinated, patient-centered care.
- Uphold infection control practices, patient safety, and respectful handling of residents and families.
Requirements
- Active LPN license in New Hampshire (or eligibility to practice in-state)
- BLS (Basic Life Support) certification
- Minimum 1 year of recent experience in long-term care or skilled nursing settings
- Ability to work evenings; guaranteed 40 hours per week on assignment
- Strong communication, documentation, and teamwork skills
- Completion of an approved practical nursing program; eligible for state licensure transfer if applicable
