Responsibilities
- Under the direct supervision of a licensed physical therapist, implement individualized treatment plans in accordance with clinical documentation, federal and state regulations, organizational policies, standards of care, and APTA guidelines.
- In collaboration with the PT, assist in developing a plan of care to promote optimal functional independence and safety; document progress in the clinical record.
- Plan for and provide skilled physical therapy treatment including restorative and maintenance techniques, patient and caregiver education; assess status and progress on each visit to ensure quality of care.
- Coordinate patient care with the multidisciplinary team, including physicians, nurses, care partners, other rehab services, and students; maintain clear communication in the clinical record.
- Monitor for changes in patient status related to the prescribed plan of care and report updates to the case manager in a timely manner.
- Maintain ongoing communication with Physical Therapists to maximize overall quality of care; complete required paperwork to reflect care provided and outcomes achieved.
Requirements
- Active Physical Therapy Assistant license in Vermont or eligibility to obtain one.
- Graduation from an accredited Physical Therapist Assistant program (associates degree).
- Ability to work under PT supervision and collaborate effectively within a multidisciplinary team in a skilled nursing setting.
- Experience in physical therapy assistance, preferably in geriatric or long-term care environments, is preferred.
- Excellent documentation practices and commitment to evidence-based standards of care; ability to stay current with HHCAHPS standards and integrate them into visit protocol.
