Responsibilities
- Assess residents with speech, language, cognitive, and swallowing concerns; establish and implement individualized treatment plans.
- Provide direct therapy services and document progress in the resident’s medical record per standards and regulatory requirements.
- Monitor patient progress, adjust treatment plans as needed, and participate in care conferences with the interprofessional team.
- Collaborate with nursing staff, therapists, and families to support communication goals and discharge planning.
- Maintain safe, patient-centered care, adhere to infection control and documentation policies, and ensure compliance with state and facility regulations.
Requirements
- Active Speech-Language Pathologist license (unrestricted) and ability to practice in the United States; Montana licensure eligibility may be required for this assignment.
- ASHACertification (CCC-SLP) or eligibility preferred.
- Master’s degree in Speech-Language Pathology from an accredited program.
- Relevant clinical experience providing services to adult populations; experience in a skilled nursing facility setting preferred.
- Ability to work in a skilled nursing facility environment and travel to the assignment location; strong communication and documentation skills.
