Responsibilities
- Conduct comprehensive speech-language assessments for residents within the long-term care setting
- Develop and implement individualized treatment plans with measurable goals
- Provide direct therapy to residents with communication and swallowing disorders
- Document evaluations, progress notes, and updates to care plans; communicate with an interdisciplinary team
- Educate residents and families on strategies to support communication and safe swallowing
- Monitor progress and adjust treatment plans in collaboration with nursing, therapy colleagues, and physicians
- Ensure documentation compliance with regulatory and facility guidelines
Requirements
- Specific license, certifications, and education requirements will be confirmed with the hiring supervisor
- Eligibility to work in the United States
- Ability to work days in a travel assignment within a long-term care setting
