Responsibilities
- Interview patients or designees to gather psychosocial information and assess the impact of illness on patients and families
- Contribute to interdisciplinary care planning; coordinate discharge planning and referrals for home or social services
- Provide therapeutic support and education to patients and families; explain medical recommendations and connect to resources as needed
- Document psycho-social assessments, plans, and progress in the electronic health record with timely, compliant entries
- Participate in interdisciplinary team meetings and patient care conferences as required
- Manage a caseload, advocate for patients, and coordinate with community resources to support ongoing care
Requirements
- Alaska licensure: LCSW at time of submittal; LMSW eligible if applicable
- Education: Master’s degree in Social Work from an accredited program
- Certifications/Licenses: Alaska LCSW license (or eligible LMSW) and required healthcare clearances
- Experience: 2–4 years in inpatient or outpatient healthcare social work
- Vaccination/clearances: Flu vaccine required; NABCS Clearance; ICPA Clearance
- Skills: Proficiency with electronic health records and common software; strong communication, confidentiality, and interdisciplinary collaboration
- Age: Must be at least 21 years old
- Ability to work in a fast-paced hospital-based behavioral health environment
