Responsibilities
- Manage complex case loads for inpatient and high-acuity patients, coordinating care across disciplines
- Develop, implement, and re-evaluate comprehensive care plans to optimize outcomes and LOS
- Collaborate with physicians, social workers, and other care teams to coordinate discharge planning and post-acute needs
- Assess patient needs, monitor utilization, and ensure appropriate resource use while complying with regulatory standards
- Document care coordination activities thoroughly in the medical record and communicate updates to the care team
- Provide patient and family education on discharge instructions, medications, and follow-up plans
Requirements
- Active RN license in the state of Florida; Nursys verification must be on profile
- BLS certification required
- Minimum 2 years of Case Management experience in a hospital/acute care setting
- Degree accreditation or 5 years of related experience required
- Ability to float to other units or sister campuses as needed
- Radius rule: within 50 miles of perm address
- Willingness to participate in a travel assignment with on-site collaboration and rapid credentialing
