Responsibilities
- Provide comprehensive nutritional care to patients identified with nutritional risk criteria, including those with special needs.
- Perform initial and ongoing patient assessments and reassessments to guide the nutrition care plan.
- Develop, implement, and document a nutrition plan of care, aligning with policy and acceptable practice standards.
- Educate patients and families on nutrition-related topics and care plans, and facilitate education across the interdisciplinary team.
- Communicate effectively with the care team to support coordinated, patient-centered nutrition interventions.
- Document all interventions accurately and in a timely manner according to policy and documentation standards.

Requirements
- Active California Registered Dietitian license and RD credential.
- Education: Bachelor's degree in Dietetics or a related field; successful completion of an ACEND-accredited program (or equivalent pathway).
- Experience: Clinical nutrition experience with assessment, care planning, and documentation; familiarity with nutrition care in a home-based or hospice setting is preferred.
- Skills: Strong assessment, care-plan development, and documentation skills; effective communication and collaboration with patients, families, and interdisciplinary teams.
- Ability to work in a home-based care setting and travel to patient locations as needed.
