The position supports patients to regain independence in self-care, daily activities, leisure and work as part of an interdisciplinary team under supervision of the Home Health and Hospice Manager.

Responsibilities
- Evaluate patients’ functional status and interpret the physician’s plan of care; develop and implement OT interventions.
- Complete initial assessments and reassessments, making decisions based on objective tests and measurements.
- Communicate observations, recommendations and treatment plans as an interdisciplinary team member; participate in team meetings to coordinate care.
- Educate patients and caregivers, provide direction to coworkers as appropriate, and document all communications and treatment activities accurately.
- Work both independently and collaboratively; protect patient rights and demonstrate empathy, especially in cases of grief, pain or trauma.
- Adapt to emergency situations, monitor for changes in condition, and engage in required education for program standards as applicable to home health and hospice.

Requirements
- Education: Graduate of an occupational therapy program accredited jointly by the AMA/AOTA; eligible for the National Registration Examination of the American Occupational Therapy Association.
- Licenses/certifications/registrations: Licensed by the State of Oregon as an Occupational Therapist; valid driver's license; current BLS certification; proof of auto insurance.
- Experience/knowledge: Knowledge of occupational therapy as it relates to physical disability, rehabilitation procedures, elderly populations, and home health/hospice concepts; continuing education is desirable.
- Travel and driving: Must have reliable personal vehicle for home visits; ability to travel to patient locations.
- Preferred: Registration as an Occupational Therapist with the American Occupational Therapy Association.
- Additional: CRASE training may be required if the travel assignment extends beyond six months.
