Responsibilities
- Assess inpatient and outpatient needs and develop comprehensive care plans across the continuum, aligning with the patient’s goals and resources.
- Facilitate seamless transitions of care, coordinating discharge planning, home health, and community referrals as appropriate.
- Collaborate with physicians, social workers, and interdisciplinary teams to optimize care, documentation, and resource utilization.
- Document assessments, plans, and progress in the EPIC electronic medical record, ensuring accuracy and regulatory compliance.
- Provide patient and family education on care plans, follow-up steps, medications, and resources to support safe outcomes post-discharge.
Requirements
- Current RN license (state licensure as an RN required)
- Minimum of 3 years RN Case Management experience
- Hospital (inpatient) case management experience required
- EPIC experience required
- Graduate of an accredited RN program
- Flu vaccine required with no exemptions
- Radius rule: traveler's permanent address must be at least 75 miles away from the assignment site
