Travel Registered Nurse Home Health Job
Registered Nurse – Home Health (Travel) in South Portland, Maine. Start date 09/21/2026 for 14 weeks, guaranteed 40 hours per week, with 8-hour shifts. Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings for care planning.
- Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health status and incorporate this data into the Plan of Care.
- Maintain up-to-date patient records with visits, contacts, problems, plans, actions, and goals; reflect changes as they occur.
- Apply infection control practices to protect staff and patients; educate patients/families on care, medications, home safety, and related topics with culturally appropriate teaching methods.
- Supervise and provide clinical directions to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Assess and reassess pain; utilize appropriate pain management techniques and educate patient and family on pain control strategies.
- Arrange referrals to appropriate agencies when needed; monitor compliance with third-party payer requirements.
- Perform on-call responsibilities and provide on-call services as assigned; maintain effective communication with patients, families, team members, and other health professionals.
- Attend interdisciplinary team meetings and other required meetings; support coordination of care across care settings.
Requirements
- Current state license as a Registered Nurse (Maine) required
- OASIS experience required
- CPR certification required
- Current driver’s license and reliable transportation required
- 1–2 years RN experience in a clinical care setting required; 2+ years home health experience preferred
- Education: Associate Degree in Nursing or Nursing Diploma required; Bachelor of Science in Nursing preferred
- Knowledge of venipuncture/IV care and home care nursing skills; ability to administer medications and manage devices as appropriate
- Ability to work independently in a home health setting and coordinate with the interdisciplinary team
