Responsibilities
- Perform clinical assessments; identify patient needs and appropriateness for therapy
- Develop, implement, and revise a Plan of Care (POC); complete progress notes and related clinical documentation
- Assess and facilitate range of motion, strength, coordination; determine need for adaptive equipment and environmental modifications; fabricate and maintain adaptive equipment and hand splints
- Evaluate patient occupational performance across self-care, work, leisure and rest; assess sensory-motor, psycho-social, and cognitive-perceptual components; consider environmental and cultural factors
- Apply infection control and universal precautions; maintain accurate records; communicate care information to physicians and caregivers while protecting confidentiality
- Accept clinical assignments within scope of education and competence; collaborate with the interdisciplinary team
- Provide education to patients and families; support safety and function in daily activities; coordinate with physicians and other health professionals
- Perform equipment troubleshooting and basic maintenance; support procurement and setup of occupational therapy tools as needed
- Perform other related duties as assigned
Requirements
- Current state license as an Occupational Therapist (OT) – Required
- CPR certification – Required
- Current driver’s license – Required
- Reliable transportation and valid insurance – Required
- Two years of experience as an OT in an acute care or rehabilitation setting – Required
- Bachelor of Science degree in Occupational Therapy from an accredited program – Required
- Ability to work in a home-care oriented environment; travel locally; flexible schedule
