This travel assignment is for a Licensed Practical Nurse working in a memory care–focused long-term care setting in South Burlington, VT. The assignment begins on April 20, 2026 and runs for several weeks with a guaranteed 40 hours per week. The schedule includes both day and evening shifts. This position is a travel - temporary assignment designed to support a continuous, high-quality standard of care for residents with memory-related needs.

Responsibilities
- Administer prescribed medications and treatments in accordance with state regulations and facility policy.
- Monitor residents’ health status, perform routine assessments, and report changes to the supervising nurse and physician.
- Assist residents with activities of daily living, maintaining safety, dignity, and independence where possible.
- Accurately document care provided, vitals, medication administration, and changes in condition in the electronic health record.
- Collaborate with the interdisciplinary team to update care plans and implement appropriate interventions.
- Adhere to infection prevention protocols, safety standards, and privacy regulations (HIPAA) at all times.
Requirements
- Active LPN license in Vermont (or eligibility to practice in Vermont) in good standing.
- Graduate of an accredited practical nursing program.
- Current CPR/BLS certification.
- Prior experience in long-term care or memory care settings preferred.
- Ability to work both day and evening shifts; comfortable in a fast-paced, resident-centered environment.
- Ability to pass facility orientation and required background checks.
