Responsibilities
- Conduct initial and ongoing assessments of home health patients to identify physical, psychosocial, and environmental needs; develop and adjust the Plan of Care accordingly.
- Implement, document, and evaluate the Plan of Care; maintain up-to-date patient records and ensure clear communication of changes.
- Deliver skilled nursing procedures (e.g., wound care, IV therapy, phlebotomy, pain management) and manage indwelling devices; administer medications per physician orders.
- Educate patients and families on care plans, medications, safety, home safety, and self-care strategies; tailor teaching to age, condition, and cultural context.
- Coordinate with the interdisciplinary team; supervise and provide clinical direction to HHA/STNA/LVN; arrange referrals; participate in team meetings; provide on-call support as assigned; ensure payer documentation and requirements are met.
- Maintain infection control practices per OSHA standards and safeguard patient confidentiality; communicate effectively with patients, families, physicians, and other health professionals.

Requirements
- Active RN license in the applicable state – Required
- Graduate of an accredited ADN or Nursing Diploma program; BSN preferred – Required/Preferred
- 1-2 years RN clinical nursing experience; 2+ years home health experience – Required
- Proof of current CPR certification – Required
- CHPN certification – Preferred
- Valid driver’s license with reliable transportation and auto insurance – Required
