Responsibilities
- Conduct comprehensive OT evaluations and devise individualized treatment plans for residents in a long-term care setting.
- Implement evidence-based therapy interventions to improve activities of daily living, mobility, fine motor function, and upper-extremity strength and coordination.
- Monitor progress toward goals, reassess as needed, and modify treatment plans to optimize outcomes.
- Document assessments, treatment plans, progress notes, and discharge summaries in accordance with professional standards and facility policy.
- Collaborate with an interdisciplinary team including nursing, physical therapy, speech therapy, social work, and case management to coordinate resident care and discharge planning.
- Educate residents and families on therapy goals, home exercise programs, safety strategies, and ongoing maintenance of gains.

Requirements
- Current Occupational Therapist license (state of practice) or eligibility to obtain licensure.
- Master's or Doctoral degree in Occupational Therapy from an ACOTE-accredited program.
- Experience in geriatric rehabilitation or long-term care settings preferred.
- Strong documentation and communication skills; ability to work independently within a travel assignment.
- Ability to work days and meet the guaranteed 36 hours per week in a skilled care environment.
