Shift: 3x12-hour day shifts (07:00–19:30)

This travel assignment is local only to the Gallup area.

Responsibilities
- Administer prescribed medications and treatments according to the care plan and facility policies.
- Monitor residents’ health status, document vital signs, and escalate changes to the supervising nurse promptly.
- Provide direct patient care including assistance with activities of daily living, mobility support, and hygiene.
- Assist with wound care, routine procedures, and routine nursing tasks within the LPN scope of practice.
- Maintain accurate and timely documentation in the electronic medical record; ensure accuracy and privacy of resident information.
- Collaborate with the interdisciplinary team to implement and adjust care plans; uphold safety and infection control standards.
Requirements
- Active Licensed Practical Nurse license to practice in New Mexico
- Current CPR/BLS certification (American Heart Association or equivalent)
- Ability to work 12-hour shifts in a 3x12 schedule
- Experience in long-term care or skilled nursing setting preferred
- Education: Practical Nursing diploma or equivalent
- Local assignment eligibility: must be available for a local Gallup, NM assignment
