Responsibilities
- Conduct resident evaluations to determine physical therapy needs and establish individualized plans of care
- Implement and adjust therapeutic interventions (therapeutic exercises, gait and balance training, mobility training)
- Monitor progress, document outcomes, and modify plans as needed in collaboration with the care team
- Provide patient and family education on home exercise programs, safety, and discharge planning
- Communicate regularly with interdisciplinary team to support ongoing care and timely transition planning
- Maintain accurate, timely documentation in the appropriate health information system
Requirements
- Active Physical Therapist license in New Hampshire or eligibility to obtain licensure
- Ability to work effectively in a skilled nursing facility setting
- Strong written and verbal communication skills and ability to document care clearly
- Availability for a 13-week travel assignment with guaranteed 35 hours per week
- Shift availability: 8:00 AM–4:30 PM, five days per week
