Responsibilities
- Assess residents’ physical therapy needs and baseline functional status
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and gait
- Perform therapeutic interventions and modalities as appropriate for geriatric rehabilitation
- Reassess progress regularly, adjust treatment plans, and document outcomes
- Communicate findings and coordinate care with the interdisciplinary team, including nursing, OT, SLP, and physicians
- Educate residents and families on exercise programs, safety, fall prevention, and post-discharge planning

Requirements
- Active Physical Therapist license in Washington or eligible for licensure in WA
- Ability to work in a Skilled Nursing Facility setting and participate in a daytime 8-hour shift
- Availability for a 13-week travel assignment beginning 11/16/2026 with 35 guaranteed hours per week
- Strong documentation skills and ability to communicate clearly with the care team
- Demonstrated adaptability to a geriatric rehabilitation environment
- Commitment to patient-centered care and safety in a SNF setting
