This assignment offers an urgent-start, increased-rate opportunity for a Physical Therapist to provide skilled rehabilitation in a long-term care setting. The engagement is travel-based, with a guaranteed minimum of 35 hours weekly and a consistent daytime schedule.

Responsibilities
- Perform comprehensive PT evaluations and develop individualized treatment plans for residents
- Implement evidence-based physical therapy interventions, including therapeutic exercises, gait training, balance activities, and functional mobility training
- Monitor patient progress, adjust treatment plans, and document outcomes in the chart in accordance with regulatory requirements
- Coordinate care with the interdisciplinary team, including physicians, nursing staff, and family/ccare providers, to support discharge planning and functional goals
- Educate patients and families on home exercise programs and safety strategies to maximize independence
- Ensure safety, proper equipment use, and adherence to infection control and documentation standards
Requirements
- Active Physical Therapist license (state-specific as applicable)
- Willingness to work in a Skilled Nursing Facility setting on a travel basis
- Availability for a 11-week assignment beginning 06/15/2026
- Ability to work the listed shift hours (08:00–16:30) and meet weekly guaranteed hours
- Experience in geriatric rehabilitation or SNF settings preferred
- Competent documentation and communication skills to support interdisciplinary care
