In this travel role, you will join a hospice care team to manage a caseload of patients, lead the interdisciplinary team, and provide compassionate symptom management and patient-centered goals of care. You will deliver care wherever patients call home—private residences, nursing facilities, or assisted living settings—with occasional outpatient visits as needed. This is a primary RN position with responsibility for coordinating care and driving patient outcomes in a home- and outpatient-based hospice program.

- Shift: 5x8 days (Mon–Fri, 8:00 a.m.–5:00 p.m. per standard schedule; occasional back-up for weekends and on-call)

- Setting: Hospice care with home-based visits and outpatient components within an outpatient/urgent care environment
- Travel assignment with primary RN responsibilities and leadership of the interdisciplinary team

Responsibilities
- Manage a caseload of hospice patients, partnering with the interdisciplinary team to establish and implement goals of care
- Conduct comprehensive assessments, monitor symptoms, and adjust plans to maximize comfort and quality of life
- Provide symptom management, administer medications, and deliver essential end-of-life care
- Educate patients and family caregivers on care plans, medication usage, and caregiving strategies
- Lead coordination of care across team members; document care in the medical record; communicate changes to the physician and other staff
- Deliver care across settings, including patients’ homes and outpatient venues, ensuring continuity, safety, and appropriate follow-up

Requirements
- Active Registered Nurse license in Colorado
- Current CPR/BLS certification
- Hospice or palliative care experience preferred; 1+ year recommended
- Ability to work Mon–Fri 8:00 a.m.–5:00 p.m. with occasional weekend and on-call coverage
- Strong assessment, critical thinking, and communication skills
- Willingness to travel within the designated area to provide home-based and outpatient hospice care
