Responsibilities
- Collects relevant data pertinent to the patient’s health or situation and analyzes assessment data to inform diagnosis and care issues.
- Develops a care plan with prescribed interventions to achieve outcomes; implements the plan and coordinates care delivery while promoting a safe environment.
- Evaluates progress toward outcomes and identifies measurable results; collaborates with patients, families, and the healthcare team to provide care.
- Delivers patient and family education; shares information with the treatment team and supports learning opportunities.
- Participates in discharge planning to ensure continuity of care and smooth transition to subsequent care settings.
- Delegates appropriately and coordinates duties of healthcare team members; performs other job-related duties as assigned.
Requirements
- Active California Registered Nurse license
- Ability to work 3x12-hour night shifts
- Eligible to work in the United States; employer participates in E-Verify
- Vaccinations required as a condition of employment; exemptions may apply
- Willingness to participate in a travel assignment and work in the listed location
- Experience in obstetric/natal nursing or hospice/home healthcare settings is preferred
