Registered Nurse – Home Health (Travel) is a 13-week assignment in Grand Junction, Colorado, supporting a Program of All-inclusive Care for the Elderly (PACE) participants in the community. Responsibilities
- Conduct comprehensive health assessments for PACE participants in the home/community setting.
- Develop, implement, and continuously reevaluate individualized nursing care plans.
- Provide direct nursing care, administer medications, and monitor clinical status per plan of care.
- Educate participants and families; coordinate preventive and rehabilitative services with the Interdisciplinary Team.
- Manage scheduling, communication, and documentation to ensure timely, accurate EHR entries and care coordination.
- Reassess participant and family needs, adjusting care plans to maintain health, safety, and independence.
Requirements
- Licensure: Current, valid Registered Nurse (RN) license in Colorado.
- Certifications: Current CPR certification.
- Experience: One to two years of recent hospital nursing or other acute care settings; home health and/or long-term care experience preferred.
- Driving and mobility: Valid driver’s license, reliable transportation, and proof of automobile insurance.
- Skills: Proficiency with electronic health record (EHR) systems and Microsoft Office; strong communication and documentation abilities.
- Setting: Ability to work effectively in a home-based, community-care environment and collaborate with an interdisciplinary team.
Education
