Responsibilities
- Administer medications and monitor patient responses within LPN scope of practice
- Perform head-to-toe assessments, monitor vital signs, and document changes in patient status
- Assist with activities of daily living and maintain patient safety and comfort
- Communicate observations and changes to RNs and physicians; participate in care planning
- Ensure accurate nursing documentation, MAR updates, and care plan adherence
- Uphold infection control, safety, privacy, and quality-of-care standards
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Ability to work 12-hour day shifts on Wednesdays and Thursdays, with every other weekend
- Strong communication and teamwork skills; reliable in a fast-paced long-term care setting
- Proficiency with electronic health records and medication administration records
- Ability to meet credentialing, safety, and compliance requirements for travel assignments
