Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as co-leader in interdisciplinary approach to patient groups.
- Deliver PT services in a home-based setting, following the established plan of care and adjusting interventions as progress is observed.
- Document patient progress and outcomes, and communicate effectively with patients, families, and the care team.
- Provide patient and caregiver education to support carryover of therapies at home.
- Collaborate with the broader care team to ensure coordinated care and timely discharge planning when appropriate.
Requirements
- Current Physical Therapist license in California (in good standing)
- Graduate of an accredited physical therapy program
- Ability to work effectively in a home-based care environment
- Strong communication and collaboration skills
- Eligible for travel assignments and able to commit to a 13-week term
