Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify needs; document findings in the clinical record and team communications.
- Implement, develop, document and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual and cultural factors; incorporate into the Plan of Care.
- Maintain up-to-date patient records with visits, problems, plans, actions and goals; reflect changes as they occur.
- Educate patients and families regarding care, medications, home safety, and self-care practices; use appropriate teaching methods.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.
Requirements
- Current Texas RN license; CPR certification required
- 1-2 years RN clinical care experience; 2+ years home health experience required
- OASIS experience required; Homecare Homebase software experience required
- Associate Degree in Nursing or Nursing Diploma program completed; BSN preferred
- Current driver’s license and reliable transportation/insurance
- CHPN certification preferred
