Responsibilities
- Provide direct nursing care for ER-level patients within a home healthcare - hospice setting, including comprehensive assessment, triage, symptom management, and acute nursing interventions.
- Administer medications, monitor vital signs, manage IV therapy, wound care, and comfort measures per physician orders.
- Collaborate with an interdisciplinary hospice team to develop and implement individualized care plans and coordinate with family/caregivers.
- Document assessments, interventions, medication administration, and education in the patient record; communicate promptly with the care team.
- Ensure safety and compliance with HLFS, infection prevention, and safety protocols; deliver patient and caregiver education on care planning and safety.
- Support transitions of care and contribute to goals-of-care discussions aligned with patient wishes and hospice objectives.
Requirements
- Active Registered Nurse license in California (state of practice) required.
- BLS and ACLS certifications (American Heart Association) required.
- Hospital Fire and Life Safety (HLFS) knowledge or certification required.
- BSN preferred.
- Acute care facility experience preferred.
- Ability to perform job duties in a home healthcare - hospice setting and to work nights on a 3x12 schedule.
