Responsibilities
- Provide inpatient pharmaceutical care for a patient population spanning neonates to geriatrics, including antibiotics and anticoagulation management, within an inpatient setting.
- Perform dosing adjustments and therapeutic drug monitoring for renal, geriatric, and oncologic patients; manage high-alert medications with safety protocols.
- Compound and dispense sterile IV medications (IV piggyback/IV push) under sterile technique; maintain accurate documentation.
- Collaborate with interdisciplinary teams (IV teams, PT, Respiratory, Radiology, Social Services, Rapid Response, Pharmacy, Transportation) and ensure timely medication-related communication.
- Document all clinical activities in the Epic electronic health record; participate in daily rounds and ensure accurate and complete charting.
- Manage Pyxis inventory and contribute to medication safety initiatives and appropriate resource utilization.
Requirements
- Active pharmacist license in hand (state or compact) at time of submittal
- Pharmacist degree: PharmD or RPh
- BLS certification
- Epic experience required; inpatient infusion experience preferred; 1 year of clinical experience minimum
- Community hospital experience preferred; travel experience not required
- Ability to work in a hospital/inpatient setting and participate in on-call rotation as described
- Pre-approved time off: 1 per contract; onboarding items/labs completed within 48 hours of contract signing
