Travel Registered Nurse Home Health Job
Registered Nurse – Home Health travel assignment in Denver, Colorado. Start date: 09/08/2026. Six-week temporary contract with 40 guaranteed hours per week, 8-hour shifts. Travel assignment in a skilled nursing facility setting.

Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors; incorporate into the Plan of Care.
- Maintain up-to-date patient records with visits, problems, plans, actions, and goals clearly documented and updated.
- Educate patients and families regarding care, medications, home safety, and other needs; use appropriate teaching methods and promote self-care practices.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service; participate in on-call coverage as assigned.
Requirements
- Current Colorado RN license required.
- 1-2 years RN experience in a clinical care setting; 2+ years of home health experience preferred.
- CPR certification required; CHPN certification preferred.
- OASIS experience required.
- Homecare Homebase software experience required.
- Education: ADN or Nursing Diploma program graduate required; BSN preferred.
- CPR certified; valid driver’s license; reliable transportation and insurance.
- Must be able to work in a home-based setting and travel locally; flexible schedule.
