Travel Physical Therapist Skilled Nursing Facility Job
Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Huntingdon Valley, Pennsylvania. This temporary role starts on 09/14/2026 for 17 weeks, with guaranteed hours of 35 per week and a 5x8 day shift (08:00–16:30). The position takes place in a long-term care, nursing home environment, delivering PT services to residents across the facility.

Responsibilities
- Conduct initial physical therapy evaluations and develop resident-centered treatment plans.
- Implement evidence-based therapies to improve mobility, gait, balance, strength, and functional independence.
- Reassess residents regularly, adjust plans as needed, and document progress in the medical record.
- Collaborate with the interdisciplinary team (nursing, OT, SLP, social work) to coordinate care and optimize outcomes.
- Educate residents and families on home exercise programs, safety, and fall prevention; promote adherence to plan of care.
Requirements
- Ability to work in a skilled nursing facility and manage an appropriate resident caseload.
- Strong communication, collaboration, and documentation skills.
- Commitment to resident-centered care and safety, with adherence to standard PT practice.
- Willingness to participate in a 17-week travel assignment and a 5x8 schedule.
- Active PT license in Pennsylvania or eligibility to practice in the state.
