Responsibilities
- Provide direct patient care to residents under supervision of a licensed nurse in a long-term care setting
- Administer medications and document administration according to care plans and facility procedures
- Monitor vital signs, assess changes in resident condition, and report to the supervising nurse
- Assist residents with activities of daily living and mobility, promoting safety and dignity
- Perform basic wound care, dressing changes, and routine procedures per policy
- Document care provided in the electronic health record and communicate clearly with the care team
Requirements
- LPN licensure in Wisconsin or eligibility for licensure (per standard care practice in a LTC travel role)
- Ability to work evenings and nights in a rotating schedule
- Comfort and experience in long-term care settings; ability to follow care plans and document accurately
- Strong communication, critical thinking, and teamwork skills
- Ability to pass background check and health screenings required for clinical deployments
- Commitment to resident safety, infection control, and professional standards
