Responsibilities
- Assess residents’ PT needs and develop individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Deliver evidence-based physical therapy interventions, including gait training, therapeutic exercises, neuro-muscular reeducation, and use of modalities as appropriate.
- Reassess progress regularly, adjust goals, and modify treatment plans to maximize outcomes.
- Document evaluations, progress notes, discharge plans, and communication with the interdisciplinary team in accordance with facility policies.
- Collaborate with nursing, OT, SLP, and medical staff to coordinate care and support safe transitions of care.
- Ensure patient safety during therapy, use proper body mechanics, and adhere to infection control and safety protocols.
Requirements
- Active Physical Therapist license in Pennsylvania (or eligibility to obtain licensure in PA)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Current CPR/BLS certification
- Prior experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to work in a fast-paced SNF setting with strong documentation and communication skills
