Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to the development and ongoing refinement of an age-appropriate, culturally relevant plan of care, engaging the resident, family, and healthcare team to achieve outcomes.
- Recommend revisions to the plan of care based on evaluation of resident outcomes and coordinate referrals as needed to address all aspects of care.
- Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; promote safety and quality in care delivery.
- Demonstrate leadership under the direction of the RN and ensure adherence to policies, procedures, and safety standards; manage exposure risks consistent with facility protocols.

Requirements
- Active New York LPN license.
- BLS certification (American Heart Association or RQI acceptable).
- Minimum 1 year of LPN experience, preferably in a long-term care setting; ability to work with adult and geriatric populations.
- Willingness to work holiday coverage and participate in day/evening/night rotation as needed.
- Ability to follow established policies and collaborate with the Interdisciplinary Team in a fast-paced environment.
